NHS South Birmingham is planning to commission a coordinating service for end of life care for patients and their carers. The overall aim of the service is to provide a single point of access and referral for healthcare professionals to a range of supportive services which will provide information, practical and emotional support to end of life care patients and their carers / families. These will be non urgent services and be part of a planned package of care. We are looking for a service provider to act as a coordinating body for access to the menu of supportive services which have already been commissioned by NHS South Birmingham as outlined below: Day support / respite for carers Pre bereavement and bereavement support End of life care Carer education and support programme The service will also act as an information and signposting service to additional supportive services for patients, carers and staff eg domestic care and benefits advice. The service will be a key element in supporting delivery of the end of life care pathway across South Birmingham, working collaboratively with specialist and generalist providers of palliative care.
