Sandwell and West Birmingham Clinical Commissioning Group (CCG) has redesigned their approach to End of Life (EOL) services to ensure delivery of an integrated and responsive model that builds on good practice and seeks to innovate and transform the care pathways for patients at end of life.

The CCG has split EOL services into three groups which will operate in synergy with each other to provide a joined up and holistic approach to EOL services for patients. Each service group will have a separate and distinct procurement approach.

The CCG recognises that the ability of the provider to meet the agreed contractual outcomes cannot be dependent on the performance or otherwise of the providers of the services in the other two groups. However, providers are expected to work together to develop appropriate relationships which will facilitate the effective delivery of outcomes. It is the intention of the commissioner to make this co-operation a condition of contract.

• Group 1 (coordination hub and urgent response service) - These services provide improved coordination of End of Life care for patients and their families. The co-ordination hub services will be supported by an urgent response team which will be available 24 hours per day 7 days per week.
• Group 2 (specialist Hospital and Community based palliative care services) – These existing specialist services form part of the EOL care pathway and are delivered by Sandwell & West Birmingham Hospitals NHS Trust (SWBH) as part of the block contract and this will continue. The service specification with SWBH will be updated to reflect other changes across the pathway.
• Group 3 (Specialist Palliative and hospice care and support services) - These services provide specialist palliative care, physical, psychological, spiritual and social support to patients, families and carers.

This tender is for the provision of group 1 services.
